| 1. |
The Ministry of Education uses the information submitted in this application
form for the issue of a teacher’s warrant in accordance with the
relevant provisions of the Education Act 1999. This information is accessed
by the Board and any personnel involved in such processing to evaluate
your application. The Board may also get information about you form certain
third parties including other government departments and authorities to
prevent or detect crime or to protect public funds in other ways as permitted
by law. We will not disclose information about you to anyone outside the
Ministry of Education unless the law permits us to. |

All data is collected and processed in accordance to the Data Protection
Act 2001, other subsidiary legislation, as well as the Privacy Policy of
the Ministry of Education, a copy of which is available on demand. |

Application should include: |

A Police Conduct Certificate covering the last six months.
Photocopies
of qualifications, which have been duly certified as being true copies
of the original by the Head of School or the Teachers’ Warrants
Board Secretary.
University transcripts of Degrees/Diplomas submitted.
Official documents certifying teaching experience claimed:
- A written declaration by the Head/s of School in the case of nonstate
schools.

Non Maltese nationals applying for a Permanent Warrant are requested
to produce a Certificate of Citizenship. You are not obliged to give this
information if you do not wish to. However, you should be aware that should
you not provide such information, the Department will not be able to process
this form. |
