Please read through the following steps to download, compile and send in the application.

| 1. |
The Ministry of Education uses the information submitted in this application
form for the issue of a teacher’s warrant in accordance with the
relevant provisions of the Education Act Chapter 327. This information
is accessed by the Council and any personnel involved in such processing
to
evaluate
your application. The Council may also acquire information from third parties
including other government departments and authorities to
prevent or detect crime or to protect public funds in other ways as permitted
by law. No personal information will be disclosed to anyone besides the
Ministry of Education, Culture, Youth and Sport unless the law permits
us to. |

| 2. |
All data is collected and processed in accordance to the Data Protection
Act 2001, other subsidiary legislation, as well as the Privacy Policy of
the Ministry of Education, Culture, Youth and Sport. |

| 3. |
Application should include: |

| |
A Police Conduct or Police Records Search
covering the last six months.
Photocopies
of ALL qualifications, declared in Section B (13 -
18). These are to include certificate of Degree and detailed transcript.
All copies are to be certified as being true copies of the original
by the Head of School or the Council's Secretary;
Approval form issued by Non State School Sector (for Church and Independent
Schools only);

Non Maltese Citizens applying for a Permanent Warrant are requested
to produce a Certificate of Citizenship. This is not obligatory, however
if such information will not be provided the Council will not be able to
process the application form. |

| 5 |
All sections of this application form are to be completed in full otherwise
the Council will not process the application. |
